Challenges and solutions in the treatment of penoscrotal lymphedema: a case analysis

Abstract

Introduction: Penoscrotal lymphedema is an uncommon condition that can cause significant deformities and functional limitations. Its main global cause is filarial infestation, although in non-endemic regions, secondary causes include surgeries and radiotherapy.Clinical case: We present the case of a 31-year-old male patient with a progressive increase in scrotal volume, reaching dimensions of 31 cm in height and 28 cm in width, accompanied by ulcerative lesions. After imaging studies, a surgical intervention was performed that included resection of scrotal tissue and reconstruction with a fasciocutaneous flap. Although the initial evolution was favorable, the patient experienced complications that required a second surgery.

Conclusion: This case highlights the importance of surgical treatment in penoscrotal lymphedema, especially in advanced cases, and contributes to the limited literature on this pathology in Peru.
