Introduction: Ureteral injuries, especially proximal ones, are challenging surgical complications that have increased with the use of minimally invasive techniques. In patients with a single functional kidney, preserving renal function is crucial to avoid additional complications.Clinical case: We present the case of a 59-year-old woman with a history of diabetes and right renal exclusion, who developed a ureteral injury after endoscopic lithotripsy. A proximal injury of 7 cm was identified, and a renal autotransplant was decided upon. During surgery, a splenectomy was performed, and an ovarian cyst was removed. The patient had a favorable evolution and was discharged 13 days later.Conclusion: Renal autotransplantation is considered a valid and safe option for the treatment of proximal ureteral injuries, providing significant benefits in preserving renal function and the patient’s quality of life.
