Introduction: Primary obstructive megaureter causes ureteral dilation without reflux. Management depends on symptom severity. Clinical case: 2-year-old girl with recurrent urinary tract infections. Ultrasound showed left hydroureteronephrosis (15 mm of ureteral diameter, 12 mm anteroposterior diameter renal pelvis). No reflux on cystography. Computed tomography revealed ureteral tortuosity. Laparoscopic extravesical reimplantation (Lich-Gregoir technique) was performed without tapering. Non-remarkable recovery. At 12 months, normal renal function and improving of ultrasound parameters. Conclusions: Laparoscopic reimplantation without tapering is safe and effective in selected pediatric cases.
